If you want to know where the "rules" governing the respect that should be shown to the US flag during the pledge of alligience you need to look in US Code Title 4. If you would like to know what respect should be shown during the national anthem, look in Title 36 section 301. It is not etiqette, it is federal law.

Barack Obama is not the only American who does not know how to pay proper respect to the flag or the national anthem, but as a presidential candidate he should. Most military or former miltary members know this, and they notice.
