INS: On Whose Side?

I'm baffled: Why is the INS, in the form of its senior officials past and present, not being held accountable for incompetence that has led, and will surely lead, to civilian deaths inside the US? Somebody please explain to me why they get a "walk" on all this, because I don't get it.
